Settlement advice

I'm in need of advice for car accident settlement. I'm trying to help my girlfriend write a demand letter. She does have her own medical insurance. Is the at fault auto insurance company going to pay her medical insurance company back? Do we only include co-pays in our demand letter? The auto insurance company has offered $6000 for past and future medical expense. The total cost charged to her medical insurance is over $6000 without complete diagnosis and treatment. She has ulnar nerve damage in her left arm with 30% loss in strength and unfortunately uses her hands as a massage therapist. As of right now treatment and how long it will last is unknown.
